Non-Invasive Monitoring in Pregnant Patient with Coarctation of Aorta Undergoing Caesarean Section

چکیده
We report a case of coarctation of aorta with previous caesarean section who underwent elective caesarean section under combined spinal epidural anaesthesia (CSEA). By continuous monitoring of oxygen saturation in lower limb, perfusion of the body below coarctation site was assessed. Good oxygen saturation in the lower limbs meant good foetal perfusion and good perfusion of vital organs. Thus oxygen saturation along with non-invasive blood pressure was monitored till regional anaesthesia effect lasted. The uneventful course of anaesthesia in the present case was related to the thorough systemic evaluation and careful anaesthetic strategy.

A Pregnant Patient with Native Aortic Coarctation and Aneurysm.
Uncorrected coarctation is rare and is known to cause hypertension in adult patients. Retrospective observational studies showed hypertension and occasionally aortic dissection to be the principal risks during pregnancy after correction of aortic coarctation. We present a case of a patient with known hypertension, who presented at 32 weeks of gestation. A saccular aortic aneurysm of 4.5 × 8.8 c...
متن کاملFetal circulation during epidural analgesia for caesarean section.
Fetal blood flow was examined during epidural analgesia in six women with uncomplicated pregnancies undergoing elective caesarean section. A non-invasive, ultrasonic technique was used to measure blood flow in the fetal descending aorta and intra-abdominal part of the umbilical vein before induction of analgesia with etidocaine and bupivacaine and 15 and 30 minutes afterwards. No appreciable ch...
متن کاملAnaesthesia for caesarean section in the presence of severe primary pulmonary hypertension.
We describe the case of a pregnant woman, 35 weeks' gestation, with primary pulmonary hypertension and coarctation of the aorta requiring emergency Caesarean section under general anaesthesia. The patient had a pulmonary artery catheter inserted before operation which revealed pulmonary artery pressures in excess of 80/40 mm Hg. These were lowered using an infusion of glyceryl trinitrate. After...
متن کاملAcquired Infrarenal Abdominal Aortic Coarctation: Treatment with Percutaneous Self Expandable Stent
Abdominal aortic coarctation is an extremely rare vascular pathology. Its etiology can be congenital or acquired. Here we present a case of acquired infrarenal abdominal coarctation in a 66-year-old woman who complained of low back and legs pain. She had no signs of resting lower limb ischemia, with diminished distal pulses and normal blood pressure in upper and lower extremities. Magnetic reso...
متن کاملAnaesthesia for Caesarean Delivery in a Pregnant with Acute Type B Aortic Dissection.
About 50% of aortic dissections in women younger than 40 years occur during pregnancy; mostly in the 3rd trimesters and postpartum period. Aortic dissection in pregnancy creates a serious mortality risk for both mother and the foetus. The ultimate goal is to ensure the safety of both the mother and the foetus. In such cases, the best method of anaesthesia for caesarean delivery is still controv...
